Passport

I know I saw it here somehwere but cannot find it. Is there a standard letter to get NJ to sign and agree so I can get my two boys passports for a trip to Canada this summer?.

I'm NCP but have 50/50 and want to apply, pay and get thier passports. I know I cannot do it without her permission (vice versa) so I would send the letter to NJ request she sign and provide a return envelope to me. Does she also have to have it notorized?

Pretty sure you both have to be present to obtain a minors passport. My ex and I had to go to the post office together and sign in the presence of the official along with our daughter.

I got the form on State Dept site. Nj doesnt have to be present but I send her the official form, she signs and has it notorized and sends it back to me and I attach to my application. I dont thnk NJ will give hard time since she can use them if needed to travel with them too.I'll just keep them with me until that day comes. We'll see how smoothly it goes.
